2012-12-13 David DeChristofaro | Board Issues Final Order

At its meeting on December 13, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors voted to suspend and place on probation the professional engineer and professional surveyor registrations of David M. DeChristofaro.



2012-12-13 Warren Drescher, DC Surveyors LLC, Carl Walker Board Accepts Settlement Agreements

At its meeting on December 13, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted settlement agreements from Carl D. Walker, P.E., P.S.; and Warren F. Drescher, P.S. and D.C. Surveyors, LLC.

Warren F. Drescher, P.S. and D.C. Surveyors, LLC, of Youngstown, Ohio, offered and provided surveying services in Ohio at a time when D.C. Surveyors, LLC’s Certificate of Authorization was expired. 

Carl D. Walker, P.E., P.S., a resident of Estero, Florida, pled guilty in Clermont County Court of Common Pleas to ten (10) counts of Having an Unlawful Interest in a Public Contract, R.C. Section 2921.42 (A) (1), a fourth degree felony, and five counts of Tampering with Evidence, R.C. Section 2913.42, a third degree felony.



2012-12-13 John Stickney | State Board Accepts Settlement Agreement in Data Theft Case

At its meeting on December 13, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted a settlement agreement from John C. Stickney, P.E.

On November 16, 2012 Governor John R. Kasich appointed Bert Dawson, P.E., P.S. of East Liverpool, to the State Board of Registration for Professional Engineers and Surveyors for a term ending September 24, 2017. Mr. Dawson previously served on the Board from 2005 to 2010. Mr. Dawson has served as the Columbiana County Engineer since 1969 and also serves as the Columbiana County Sanitary Engineer. Mr. Dawson has a B.S. from Youngstown State University and an M.S. from Carnegie Mellon University. In 2005 Mr. Dawson was named Urban County Engineer of the Year by the National Association of County Engineers and County Engineer of the Year for 2012 by the County Engineers Association of Ohio. Mr. Dawson is a Registered Professional Engineer in Ohio, Pennsylvania and West Virginia and a Registered Professional Surveyor in Ohio.

At its meeting on September 27, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted settlement agreements from Patrick Fiedler, P.E. and Fiedler Group, Douglas R. Alvine, P.E. and Alvine and Associates Inc., John M. Hortel, P.E. and Robert G. Compton, P.E.

Patrick Fiedler, P.E. and Fiedler Group of Los Angeles, California, offered and provided engineering services in Ohio at a time when the firm did not possess a Certificate of Authorization.

Douglas R. Alvine, P.E. and Alvine and Associates, Inc. of Omaha, Nebraska, offered and provided engineering services in Ohio at a time when Mr. Alvine did not possess a certificate of registration and Alvine and Associates, Inc. did not possess a Certificate of Authorization.

John M. Hortel, P.E., a resident of Chillicothe, Ohio, aided and abetted a firm in its offering and providing of engineering services in Ohio at a time when the firm did not possess a Certificate of Authorization.

Robert G. Compton, P.E., a resident of Fort Thomas, Kentucky, engaged in the unlicensed practice of engineering and represented himself as a professional engineer in Ohio at a time when Mr. Compton did not possess a registration with this Board to provide engineering services.

At its meeting on July 19, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted settlement agreements from Elizabeth Hanson, P.E. and Delbert R. Ogle, P.E.

Elizabeth L. Hanson, P.E, a resident of Massillon, Ohio, was randomly selected for the Board’s continuing professional development audit. Ms. Hanson contacted the Board and reported that even though she indicated on her renewal form that she met the continuing education requirement, she did not complete the requirement prior to renewing her registration for 2012.

Delbert R. Ogle, P.E, a resident of Cincinnati, Ohio, engaged in the unlicensed practice of engineering and represented himself as a professional engineer on 30 projects at a time when Mr. Ogle’s professional engineer registration was expired.

Thomas A. Bellace, P.E., a resident of Houston, Texas, was charged with violations of Ohio Revised Code Sections 4733.20 (A) (5) and Ohio Administrative Code Sections 4733-19-01 and 4733-35-08 based on his voluntary surrendering of his professional engineer license in Florida and subsequently the North Carolina Board’s refusal to renew Mr. Bellace’s professional engineer license. Mr. Bellace failed to report both actions to the Ohio Board.

At its meeting on May 24, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors voted to revoke the professional surveyor registration of David A. Holden.

David A. Holden, a resident of East Canton, Ohio, was the subject of several complaints received by the Board involving surveys that Mr. Holden performed. The Board made several attempts to contact Mr. Holden and made repeated requests for a response to the complaints. Mr. Holden failed to respond to any of the Board’s requests.

At its meeting on May 24, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted settlement agreements from the following companies and individuals: Engineers Northwest Inc.; U.P. Engineers & Architects; James Westbrook & Associates, LLC; Innovative Solutions Unlimited, LLC; Consulting Engineers, Corp.; Frank A. Hammer; William T. Novak; Paul R. Ripple; Christopher B. Stutz; Paul G. Keffler; Jirimiah Conkle; Richard J. Mazur; Jeffrey P. Miller; Enrique I. Tabak; and Bemba K. Jones.

Engineers Northwest Inc. and Robert C. Raichle, Jr., P.E. of Seattle, Washington offered and provided engineering services in Ohio without first obtaining an Ohio certificate of authorization.

U.P. Engineers & Architects, Inc. and George Kiiskila, Jr. of Houghton, Michigan offered and provided engineering services in Ohio without first obtaining an Ohio certificate of authorization.

James Westbrook & Associates, LLC and James P. Westbrook, P.E. of Duluth, Georgia failed to renew their certificate of authorization to practice engineering in Ohio which expired on June 30, 2010.

Innovative Solutions Unlimited, LLC and Frank Barbarits of Piketon, Ohio did not renew their certificate of authorization to practice engineering in Ohio. The firm continued to offer and provide engineering services before reinstating their certificate of authorization.

Consulting Engineers Corp. and Tracy S. Mitchell, P.E. of Cincinnati, Ohio offered and provided engineering services in Ohio without first obtaining an Ohio certificate of authorization.

Frank A. Hammer, P.E, a resident of Uniontown, Ohio, was randomly selected for the Board’s continuing professional development audit. Mr. Hammer contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

William T. Novak, P.E., a resident of Athens, Ohio, was selected for the Board’s continuing professional development audit. Mr. Novak failed to respond and provide documentation as required by Ohio Revised Code 4733.151.

Paul R. Ripple. P.E., a resident of Youngstown, Ohio, was randomly selected for the Board’s continuing professional development audit. Mr. Ripple contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Christopher B. Stutz, P.E., a resident of Navarre, Ohio, was randomly selected for the Board’s continuing professional development audit. Mr. Stutz contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Paul G. Keffler, P.E., a resident of Youngstown, Ohio, was randomly selected for the Board’s continuing professional development audit. Mr. Keffler contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Jirimiah Conkle, P.S., a resident of Westerville, Ohio, was randomly selected for the Board’s continuing professional development audit. Mr. Conkle contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Richard J. Mazur, P.E., a resident of Waterville, Ohio, was randomly selected for the Board’s continuing professional development audit. Mr. Mazur contacted the Board and reported that he could not provide evidence that he completed his continuing education hours.

Jeffrey P. Miller, P.E., a resident of Norton, Ohio, was randomly selected for the Board’s continuing professional development audit.  Mr. Miller contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Enrique I. Tabak, P.E., a resident of Toronto, Ontario, entered into a consent order with the Texas Board of Professional Engineers wherein he agreed to cease and desist from all engineering practice in the state of Texas as a result of allegations that he designed and sealed plans for a project that was not prepared in a careful and diligent manner and the certification was misleading.

Bemba K. Jones, P.S., a resident of Cleveland, Ohio, misrepresented pertinent facts concerning employees, associates, consultants and joint-ventures by listing individuals and firms not affiliated with his firm on the Bemba K. Jones, P.S. & Associates website without their knowledge and permission.

At its meeting on March 15, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted settlement agreements from Jason Kabbara, P.E., Frederick Hargrove, P.E., James Geeslin, P.S., Lynn Congos, P.E., Browne & Associates, Inc. and Paul Boeshart, P.S.

Jason Kabbara, P.E., a resident of Clinton, Ohio, applied his seal and signature to engineering plans for which he did not have personal professional knowledge and direct supervisory control and responsibility.

Frederick Hargrove, P.E., a resident of Cincinnati, Ohio, applied his seal and signature on engineering plans for which he did not have personal professional knowledge and direct supervisory control and responsibility.

James Geeslin, P.S., a resident of Coldwater, Ohio, pled guilty to a felony OMVI in Mercer County Clerk of Common Pleas Court and was found by the Celina Municipal Court to have caused excessive crop damage to an adjoining land owner while performing a survey for the adjacent property.

Lynn Congos, P.E., a resident of Columbus, Ohio, was selected for the Board’s continuing professional development audit. Congos failed to respond and provide documentation as required.

Browne & Associates, Inc., located in Lisbon, Ohio, failed to renew their certificate of authorization to practice surveying in Ohio which expired on June 30, 2009.

Paul Boeshart, P.S., a resident of Hebron, Ohio, engaged in the unlicensed practice of engineering when he designed a storm sewer system and parking lot improvements for a project located in Newark, Ohio.

At its meeting on March 15, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors voted to revoke the professional engineer registration of Robert F. Keller, P.E.

2012-01-19 Beech Engineering & Manufacturing, William Bruce Ingram, Yog Paul Singhal, Daniel Walch | State Board Accepts Settlement Agreements in Three Cases

At its meeting on January 19, 2012 the Ohio State Board of Registration for Professional Engineers and Surveyors accepted settlement agreements from Beech Engineering & Manufacturing, William Bruce Ingram, P.S., Yog Paul Singhal, P.E. and Daniel Walch, P.E.

Beech Engineering & Manufacturing located in New Philadelphia, Ohio entered into a settlement agreement wherein the firm agreed to permanently remove Engineering from its name on or before June 1, 2012 and will not offer or provide engineering services until the firm has been granted a certificate of authorization from the Board.

William Bruce Ingram, a resident of Amanda Ohio, was selected for the Board’s continuing professional development audit. Ingram contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Yog Paul Singhal, a resident of Huber Heights, Ohio, was selected for the Board’s continuing professional development audit. Singhal contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

Daniel Walch, a resident of Toledo, Ohio, was selected for the Board’s continuing professional development audit. Walch contacted the Board and reported that even though he indicated on his renewal form that he met the continuing education requirement, he did not complete the requirement prior to renewing his registration for 2011.

2012-01-19 Planning and Design Services Inc | State Board Issues Final Order

In September 2011 Planning and Design Services Inc., a firm located in Zanesville, Ohio, was charged with violations of Ohio Revised Code Sections 4733.16 and 4733.20(A) (1) and (5) for providing surveying services in the State of Ohio at a time when the firm’s Certificate of Authorization was expired.
